By signing this application form, I give permission to the Implementation Team to release my completed Record of Assessment to MiHR for review and evaluation. MiHR represents that the information will be kept confidential in accordance with privacy legislation. By signing this application form, I agree that in no event shall MiHR, its subsidiaries, affiliates, members of the governance structure, employees, or members of the mining industry who participate in the governance, management, administration or implementation of the system be liable to me or any other person or entity in any manner (including negligence) for damages of any kind including, but not limited to, damages arising from the actions or inactions of any person certified by MiHR.
